During certain social aspects of my life, I have met many people with a weight problem. All but one of those have a real problem with crippling anxiety and depression. This is an area I have spent a lot of my adult life involved in. The thoughts of those with such illness have more pressing issues on their minds than exercising. Some finding it difficult to think of anything other than death, others so scared of social interaction, fear keeps them prisoners in their own homes for years. Of course this will have a massive impact on fitness. There are people who are so low in mood that food is the only thing that brings pleasure.
I'm not saying all obese people are the same but I'd lay my life on it being far more than not.
All anyone can ever see is the fat and lazy though eh, it disgusts you? It insults you? Guess what, tough shit. It is YOUR problem.

The underlying problems did exist before, but food was too scarce to have the problem actually materialise.
And even with exercise and healthy food, one can be heavy. I weigh about 240, 6 feet and a bit long. But I cycle, run 25 miles a week, do not overly eat. -
