If you recently had your ass kicked, please copy this form to your user page and fill in the answers to the following questions.

This looks strangely familiar.

This looked way better in its original format. I’ve had this in my notes forever. Think I pulled it off of turfwars wiki. If I recall correctly brown note created this or had a hand in it. He used to link it whenever noobs would complain about an ass kicking in forums 😂
