I was at work tonight and one of my patients was moaning loud enough to hear her at the desk from the end of the hall through a closed door. The patient can't answer questions consistently or reliably as she has had numerous strokes. The family refuses to allow her to be given morphine or Ativan. I tried talking to them and explaining why I believe she needs medication. They still refuse. And to make things worse, there are 3 patients/families like this on this floor tonight. Aaaaaaaggggggrrrrrrrhhhhhhh!

Why would they cruelly let her suffer like that? Religious issue or something I'm just not thinking of? I guess I don't understand the downside to helping someone be more comfortable. I feel bad for the patient.

So what? Perhaps she told her family not to give it to her ahead of time. Wasn't your call regardless of whether you felt you were right or not. Either way I'm sure it wasn't that bad. Some kid in Africa just died of starvation. You were annoyed by sounds.

We had to rapid tranq a pt kicking off yesterday ~ from one extreme to the other. Quality of life etc etc

You are trained to do what is best for the patient. Hospice means comfort or palliative care. To be unable to provide that care and having the patient moan so loud that everyone on the floor can hear her horrible suffering must be truly awful. Frustrating. Stressful. Sad. Even angry feelings.
Maybe try to explain that the morphine will allow her to sleep comfortably. No morphine she moans which is a very clear indicator of patient stress, not allergy. Perhaps they would allow a test of your theory. Just to try it out...?
Bottom line is all you can do is your best. Hang in there. Be glad that most loved ones are more sensitive to clear and obvious signs of stress and discomfort in their family member.

Sorry if I offended anyone... I just don't see how this still was anyone's call but hers or her loved ones. Id hate if I decided to not takes meds, and so because I didn't do what was suggested, I'd have to deal with a person trying to stop what I arranged and be in pain on top of that. I'd be disappointed.

That sucks. It's amazing how little regard our soceity has for dying or terminally ill people.
My peeve is when a DNR order is treated as "do not treat" and they don't put a cannula on the Pt to get their sats up...instead they write them off and watch them slowly expire

Its hard when someone you love is passing away, I watched some one I loved dearly pass away from cancer and it was on of the worst experiences of my life. When I reflect on on it, I got to spend invaluable time with him, that I wouldnt of got if he had passed away quickly, I made sure to say everything I needed to say. To know your helpless is the worst feeling in the world

We are kinder to are pets that are ill and old, we put them to sleep so they don't have to suffer any more pain. I've beat cancer once and don't wish that on anyone.

A DNR, mine at leest is talord. I dont want heroic meshurs. But if i hav an infectn, treet. Pain meds and salvag treetmnt tto keep me comfrtbl, an its all bout brain waves fr sherrys sake. If ther IS any, treet. If not, no plugging. But thay wont starv me, as she thnks that is painful aftr my coma. I didnt no how to eet and def had alot of pain.
Its tuff waching a lovd one die, im vry conshus of sherry an th rest of my wondrfl famly. Its not mcch fun being th patient eethr, an vrry imprrtnt to get thees desisns made long bffor y,u hav to.Prayrs fr ths lady

We all must get our own living wills filed with our families, physicians and lawyers clearly detailing our desires. Talk about it with several family members to make sure they understand what each detail means so your desires are known and emphasized.
This thread is a great reminder.

I feel like I'm missing something...I'd hate to give the impression of not having compassion for people suffering, because I absolutely do. I feel like I got the impression this is something she talked over with her family before ..why are the family refusing pain meds?

I kind of assumed you'd never been in this situation as a caregiver of a patient or loved one at end of life so weren't really understanding the situation. Was trying to give you the benefit of the doubt.
Extremely loud moaning is a pretty clear indicator of patient distress and discomfort imo.
Even if *you* don't wish to be made very comfortable and pain free at end of life, most of us do wish that kind of comfort care.

I work in a hospital and am lucky I never hear someone moaning with pain.
