1. It's the money you have to pay to keep the cops off your back.

If you don't want to pay it then
A - buy global protection from the don for 5 respect points, or
B - use a consiglieri (a loot item) or
C - go to the missions tab and pay for protection for each city individually. (Note that you will still be "busted" occasionally with this option).

Still have to pay in all cities they aren't capo and still will be busted doing missions.
