It's handled automatically. You can use one weapon, melee and armor for each mob member. Check out the thread titled how to arm your mob.

If some don't auto equipped, you're too low of a lvl to use it. It will equipped when you reach the lvl need.

For equipping armour piercing ammo or hoods visit your profile and click manage inventory

Yea, some you can only equip manually or use it against otyers. Find the post explaining all of the loot items.
