On Monday I am going to get my iPhone replaced at apple. How do I go about transfering my account to the new iPhone when I get it?

Just sign in with your old account info. That's all I did.

But how do I sign out of the account on the iphone that im giving in so that if they decide to use that as a replacement for someone else that they cant use my account?

Dont rely on whoever you giving the phone to do anything. Reset the phone yourself before turning it in.

any specific details I should give Nick (like imei etc) when asking to log me out of the old phone?

Change your password on line, then you'll sign out of TW dk on the new phone an sign in with the new info.
