Seems like there should be some consequences for getting knocked into the ER, I know it's to help players who are offline when they get attacked, but maybe suspension of income or pay a fine to get out.

Well part of the "penalty" of being in the ER is that you can't attack either, which is crucial if you are fighting in a vendetta (if you are in the ER in a vendetta you don't contribute to vendetta attacks or defense). Another penalty is that you do have to pay to heal up to get out of the ER or you have to wait a while before you heal normally, which keeps you outta commission during that time.
